Skilled Nursing/RNs perform patient evaluations, including monitoring and tracking vital signs, clinical procedures such as IV placement, phlebotomy, and administering medications. In the home, your nurse monitors your progress and keeps in contact with your physician to make sure that your treatment protocols are appropriate.
Intravenous Medication Administration
Physicians order the type of infusion, duration of care, and prognosis and our nurses implement the plan for clients requiring treatment and intravenous medication at home. Skilled nurses visit as often as needed to teach, implement, clean, change, and adjust all types of IVs and access ports .

Nurses assess, monitor and teach patients to ensure they are taking medications appropriately, and understand what they are taking and why. They can also teach the patients and families to set up medication boxes to ensure safety and compliance.
Rehab Home Health offers a comprehensive, interdisciplinary behavioral health program offering Behavioral Health Nurses and treatment team meetings led by local psychiatrists. It is designed to assist patients with the psychosocial aspects of illness and aging. Comprehensive evaluations are provided and treatment plans established to ensure that mood and mental status, symptom management, medication effectiveness, safety and function, and appropriate level of care are all addressed. They provide education and support for the family and caregivers and ensure that the legal and financial needs of the patient and family are considered.
