Home Health services include healthcare management and clinical experts that will provide uncompromising levels of attention, customer service and integrity. Rehab Home Health’s team of licensed clinicians will work with patients and physicians to develop a plan of care that will meet each individual’s specific needs. We are a Medicare Home Health Agency and  Medicare & private insurances are accepted.

Skilled Nursing/RNs perform patient evaluations, including monitoring and tracking vital signs, clinical procedures such as IV placement, phlebotomy, and administering medications. In the home, your nurse monitors your progress and keeps in contact with your physician to make sure that your treatment protocols are appropriate.

 

Nursing Services and Programs

Intravenous Medication Administration
Physicians order the type of infusion, duration of care, and prognosis and our nurses implement the plan for clients requiring treatment and intravenous medication at home. Skilled nurses visit as often as needed to teach, implement, clean, change, and adjust all types of IVs and access ports .

Infusion Therapy Administration
Physicians order the mode of infusion, duration of care, and prognosis and our nurses implement the plan for clients requiring treatment and intravenous medication at home.

Wound Care
Nurses specializing in wound care visit as needed to teach proper wound care and to ensure proper treatment and healing of all wounds.

Diabetes Management
Our experienced nurses provide help with all aspects of managing diabetes including: blood glucose monitoring, insulin administration and tracking, exercise plan, menu planning, education and more.

Chronic Illness Management
Nurses continually assess the client and routinely communicate with the client’s physician to provide optimal care.

Alzheimer’s Care/Dementia Care
Our nurses work with the client’s family to educate on medication compliance and to help create a comfortable environment including simple routines and visual cues as a positive substitute for diminishing mental processes.

Medication Management
Nurses assess, monitor and teach patients to ensure they are taking medications appropriately, and understand what they are taking and why. They can also teach the patients and families to set up medication boxes to ensure safety and compliance.

Behavioral Health Program

Rehab Home Health offers a comprehensive, interdisciplinary behavioral health program offering Behavioral Health Nurses and treatment team meetings led by local psychiatrists. It is designed to assist patients with the psychosocial aspects of illness and aging. Comprehensive evaluations are provided and treatment plans established to ensure that mood and mental status, symptom management, medication effectiveness, safety and function, and appropriate level of care are all addressed. They provide education and support for the family and caregivers and ensure that the legal and financial needs of the patient and family are considered.
